Well Permits

The permitting process consists of two parts. The first is a request for a site evaluation and the second part is the application for a well permit. These forms can be downloaded from the links shown or we can mail them to you.

Complete both the site evaluation and the application for a well permit. Send them to the Western Upper Peninsula Health Department to the address shown on the form. Enclose the fee ($240) as indicated in the instructions.
